The ZAO, recently required certain details from DDOs / AOs with regard to revision of pension of pre 2016 retirees .  The ZAO for this purpose prescribed a Form which is posted hereunder.  If you retired before 2016, you are requested to furnish the details to the DDO / AO of the office from which you retired with supporting documents.
