Revision of pension Details to be furnished to AO

The ZAO, recently required certain details from DDOs / AOs with regard to revision of pension of pre 2016 retirees.  The ZAO for this purpose prescribed a Form which is posted hereunder.  If you retired before 2016, you are requested to furnish the details to the DDO / AO of the office from which you retired with supporting documents. If assistance is required in deriving the PAY at various dates, you may approach the Secretaries / Contact person.
